General: Based on interview, the following was found: 1) On 05/04/2022, a three-year-old child was given a shower while at the facility. Staff stated that the child's clothes and slippers were very soiled and smelled. Staff also stated that the child's hair was dirty and her body smelled so staff asked the child if she would like a shower and the child said, "yes". There were 2 staff present during the shower, which is located upstairs in the facility. Staff stated that all of the child's clothes were removed and 1 of the staff members used Baby Suave Shampoo and Body Wash on the child's hair and entire body including her vagina. Staff stated that a wash cloth was used on the child's body. Staff stated that the child's clothes and slippers were laundered. The parent of the child found out about the shower after the child mentioned it at pick up time. The parent was not notified prior to staff giving the child a shower. 2) On more than one occasion, a one-year-old child, who is the sibling of the three-year-old child, was given a bath in a classroom sink. Staff stated that on more than one occasion the one-year-old child had an odor to him, was visibly dirty and his clothes were dirty. All of the child's clothes were removed when the bath was given to the one-year-old child. General: Based on interview and review of documentation, the surveyor found that on 11/23/2020, a two-year-old child was chasing a ball in the gym, rolled over the ball, and hit his head on the corner of a doorframe. As a result, he received a gash above his eyebrow and a bruise on his cheek. The staff person was alone with nine children in the gym when the incident occurred, and could not leave the gym to take the child to the restroom. Therefore, she called the staff person in charge, and found she was not in the building, but running an errand. The staff person stated that she was unaware that the staff person in charge was not in the building when she called. The staff person stated that she had to hold her bare hand over the wound because blood was streaming down the child's face, and there were no gloves or Band-Aids available in the first aid kit. While the staff person waited for the staff person in charge to return, the staff person in charge called a staff person working in a toddler classroom for assistance. This staff person entered the gym, escorted the child downstairs, applied gauze, and notified the child's mother. The child’s mother transported him to the hospital where he received two stitches. In short, there was a lack of communication between the staff person in charge, and the staff person caring for children in the gym. The staff person in charge had to call another staff person for assistance. In the meantime, the staff person had to use her bare hand to cover the wound because there were no gloves available. TYPE B CITATION: Based on Observation, review of documentation and interview, the facility failed to meet this regulatory requirement. On 3/5/14 the director reported to the DRCC office an incident that occurred at the facility on 3/4/14. The director reported that a staff person at the facility (hire date: 10/7/13) shut five (5) children in a closet in the gym because she was frustrated with the children and commented to the staff person working with her at the time, "call me mean, but they need to learn". The surveyor toured the gym and closet areas during the investigation and found that the closet contained mobile basketball goals, archary targets and a large metal ladder. There is no light source in the closet; therefore, it is dark inside the closet when the doors are closed.
