General: Based on interview and review of documentation, it was determined that the center failed to ensure the confidentiality of child-specific information, as required by 922 KAR 2:090. A staff member in charge communicated to a parent that their child had been bitten and sent a photograph as documentation. It was later discovered that the photograph depicted a different child. The staff member stated that both children are the same age and look very similar. A follow-up message was sent to the parent clarifying that their child was not bitten and included a corrected photo.

General: Based on observation, the following was found: One-Year-Old Classroom A fan on a low table was accessible to the five (5) children in care. Infant Classroom 1) One (1) uncovered outlet was in the infant/ toddler play space. 2) A bouncy seat was stored in a crib. 3) Bins of toys, boppy pillows, a staff member’s purse, and other miscellaneous items were stored in a second crib. 4) Three (3) giant Rock-A-Stack toys were in a third crib. *The storage of items on bedding promotes the transfer of dirt and grime. Common Area Four (4) brooms and Four (4) dustpans were adjacent to classroom doors; therefore, the items were accessible to the children in care. Three-Year-Old Classroom (big room) 1) A large set of folding wall partitions on a rolling cart was next to the children’s cubbies. At approximately 10:18 am, the surveyor observed a female child climbing on the edge of the cart. 2) A highchair was observed in the classroom. It should be noted that the children present in the classroom were ages three to eleven (3-11) years old. The surveyor inquired about the highchair, and a staff member stated that they often have to put a two-year-old autistic child in the highchair for approximately ten (10) minutes to keep him from running out of the classroom. 230 - Volunteer/Board Member Compliance Not Applicable 922 KAR 2:090. General: Based on observation, the following was found: Three-Year-Old Classroom (big room): The surveyor entered the classroom at approximately 10:18 am and observed thirteen (13) children ages three to eleven (3-11) years old and two (2) staff members. One (1) staff member was chasing children and repeatedly telling them to sit down while another child was climbing on a wall partition cart. The staff member appeared extremely frustrated. The other staff member was attempting to conduct circle time, but the children were restless and not engaged. There were two (2) school-age children present who were standing by the window watching the children run around the classroom. The surveyor found limited programming materials. For example, there was one (1) box of Legos, a few blocks, three (3) dress up clothes, two (2) cars, one (1) bag of Legos, two (2) baby dolls, two (2) pots (but no dramatic play kitchen), a few board games, and some canisters of playdough.
