The surveyor noted that the strap was not fastened around the child. The staff person stated that the child was sitting on the strap. (2) The surveyor observed a large box fan that was positioned on a low shelving unit in the Silly Seahorse classroom. The surveyor noted that the fan was in operation and within reach of the toddlers present in the classroom. The surveyor asked how the staff ensure that a toddler doesn't stick their fingers into the fan. A staff person replied, "They know they are not supposed to touch it." A review of a child's medication form in the younger infant classroom found that the parent requested Tylenol be administered to their child on 8/12/14, 8/19/14, 8/21/14 and 8/25/14; however, the directions written by the parent did not include specific times for the medication to be administered. The parent wrote "as needed" on the form. Staff working in the classroom stated during interview that if the medication was administered it would be documented in the child's notebook that goes back and forth between home and school. Staff stated that the child transitioned up to the older infant classroom; therefore, is no longer enrolled in the younger infant classroom. The surveyor reviewed the child's notebook while touring the older infant classroom and found that it did not include medication documentation from August 2014. The staff person in charge stated that when a child's notebook gets full, some of the pages are torn out and sent home with the child's parent. Therefore, the facility did not obtain specific times for the medication to be administered and did not keep a written record of the administration of medication.
