The surveyor noted that the strap was not fastened around the child. The staff person stated that the child was sitting on the strap. (2) The surveyor observed a large box fan that was positioned on a low shelving unit in the Silly Seahorse classroom. The surveyor noted that the fan was in operation and within reach of the toddlers present in the classroom. The surveyor asked how the staff ensure that a toddler doesn't stick their fingers into the fan. A staff person replied, "They know they are not supposed to touch it." The staff person was in the process of locating the mother's phone number when the mother called the facility and stated she just realized that the child was not with her. The staff person in charge reviewed the facility's security footage and found that the child was left alone outside for twenty-one (21) minutes. The staff person in charge stated during interview that she did not report the incident to the cabinet.
