General: Based on review of documentation, the following was found: 1. Staff member (DOH: 11/17/25) had a tuberculin skin test result on file dated 02/06/24, that exceeded two (2) years and therefore was no longer current. 2. Staff member (DOH: 11/17/25) had a tuberculin test skin result on file dated 02/04/24, that exceeded two (2) years and therefore was no longer current. 3. Staff member (DOH: 01/20/26) did not have a tuberculin test skin test result or a statement from a health care professional stating they were free of active tuberculosis on file for review at the time of the inspection. 4. Staff member (DOH: 03/05/26) did not have a negative tuberculin test result or a statement from a health care professional stating they were free of active tuberculosis.
