It was found that Eddie's Southern Style BBQ Catering Company supplies breakfast and lunch for the facility. However, there was not a copy of the catering permit issued by the health department on file at the facility. General: Based on observation and interview, this regulatory requirement was not met. Upon arrival at the facility the surveyor asked the director how many children were enrolled at the facility. The director stated only one (1) child is enrolled but the child was not at the facility that day. However, there was a two-year-old child and a seven-week-old infant present at the facility. The director stated that the two-year-old child was her nephew and the infant belonged to a friend of hers. The surveyor asked if enrollment information was on file for each of these children and she indicated it was not. In addition a staff person from First Steps came to the facility to provide services for the two-year-old child. There was no written permission from the parent for third party professional services in the child care facility. General: Based on observation and interview, it was found that this regulatory requirement was not met. During an interview with a staff member, it was determined that the children from the preschool room would be going into the toddler classroom to use the restroom. This is so the other restroom in the facility could be used as the changing area for the infants who attend the facility since the infant room does not have a hand-washing sink. Since the preschool children would be using the restroom in the toddler room, this would cause an issue with the traffic pattern in that classroom.
