General: Based on interview and review of documentation, on 04/14/26 at approximately 3:45 pm a five-year-old child absconded from the Preschool Classroom door that exited to the outside of the building and ran to the front of the building, and entered the roadway in front of the building. The staff in the classroom was the only adult present and therefore yelled out to the staff members on the playground for help. Two (2) staff members ran after the child but did not catch up to him until he had already entered the roadway. All cross traffic came to a stop and the child was unharmed. The child was returned to the classroom and the parents were notified by aide of an interpreter. The parents informed the center the child had a similar incident at home where he had absconded from the home and ended up in the roadway. An incident report was written and signed by staff and the child's guardian and the child remained enrolled at the center at the time of survey.
