During the tour of the facility, children were observed going into the toilet room and back bedroom, at several different times. The provider remained in the kitchen area during these times. The children were not within scope of vision to the provider. One (1) child was observed going through a low gate and into the toilet room. The child was observed shutting the door to the toilet room. The child remained in the toilet room for approximately one (1) minute, while the child washed his hands. The provider remained in the kitchen area, preparing food for lunch. The provider stated that she thought the older children had to be supervised through scope of vision or range of voice.
