During a tour of the multipurpose room/cafeteria, the surveyor observed a fire extinquisher sitting on the floor and accessible to the children. General: Based on Interview, the facility failed to provide for the safety and comfort of each child. On 1/10/12 the director reported to the DRCC office an incident that occurred at the facility on 1/3/12. The director reported that a staff person at the facility (hire date: 5/17/97) smacked a three (3) year old child on his hand/arm and asked the child "how does that feel?" in response to the child being uncooperative to the staff person's requests to lay quietly during naptime. The director further explained by stating that the child threw a tantrum and became aggressive by hitting and scratching at the staff person's hands/arms as he was trying to resist the staff person's restraint.
