General: Based on Interview, on January 14, 2014 it was determined that the water in the building had low pressure due to a water main break at the rear of the building. Interview further revealed that the water to the childcare center was manually turned off on January 14, 2014 so maintenance could repair the break. Interview also revealed that on January 15, 2014, children were allowed in the center that still had no running water to receive care at 7:00 A.M. Interview of numerous staff revealed that the children could not flush the toilets leaving feces and urine in the toilet bowls. Further interview of staff revealed that the children could not wash their hands properly after using the bathroom and that the staff was prompted to instruct the children to use hand sanitizer and baby wipes as a disinfectant for their hands. General: Based on interview and review of documentation, a child was lifted by her wrists and then placed in a staff member's lap where the staff member put her arms around the child's arms, restricting the child's movement for approximately four (4) minutes. Then the staff member grabbed the child's forearms and sat her down beside the staff member for ten (10) minutes before the child slid to the floor. After several minutes, the staff grabbed the child and carried her by the wrists to a cot where she was placed while the other children were playing. The staff member placed her leg and foot over the child's body while the child was on the cot. The staff member then used her foot to scoot the cot closer to her several times. The child remained on the cot for at least 25 minutes prior to being allowed up to rejoin the other children.
