Two children were observed banging on the window, then jumping on a cot while both staff members were trying to set up cots and mats for naptime. One child was observed on his knees in a chair then fell out of the chair which ended up on top of him, at this time three staff members had their backs turned. In the Infant room, a baby that was being held by a staff member dropped a pacifier on the floor. General: Observation of the infant room revealed eight (8) children in care with one staff member. Interview with the staff member revealed the youngest child was three (3) months old. A second staff member arrived in the infant room within a minute of the DRCC representative entering the room. Prior to the second staff member entering the classroom, the minimum staff to child ratio per regulatory standard was exceeded by three (3) children. Review Division of Regulated Child Care revealed a Directed Plan of Correction (DPOC) that was issued to and signed by a representative of the child care center on June 15, 2022. General: Observation of the child care center revealed the following: 1. In the infant room a staff member was yelling out a window onto the outdoor play area at a child saying "I am going to beat your a**". Interview with the staff member in the infant room revealed the child being yelled at was their own child. There were eleven (11) children present on the outdoor play area and ten (10) infants present within the infant room at the time of the incident. 2. Throughout the duration of the inspection a staff member in the one-year-old room was being very loud when speaking to the children. At times the tone of voice used by the aforementioned staff member was not appropriate. During observation of the classroom he/she used a loud tone, telling the the children "Lay down" and "Wait a minute". The staff member then very loudly told a child that was crying to "Stop". Also, he/she used a very, loud voice when calling a child's name and giving them directions of "Move, get up and lay down". General: The environment in the Preschool Room was chaotic on 07/06/2022. There were twelve (12) children present with one (1) staff. The children ranged in age from four to five years old. The children were observed yelling very loudly and screaming, making it difficult for the surveyor and staff member to hear each other in close proximity. Children ran around the room, throwing toys across the room and at each other. On at least six (6) separate occasions the children were climbing on the top of tables and jumping down off the table with no redirection from staff. A four-year-old child was observed climbing on top of stacked chairs to reach the top of the bookshelf. Children ran throughout the room and pushed and shoved each other. There were multiple toys on the floor, creating tripping hazards. There was no sort of engaging programming observed to be offered to the children. The staff member supervising the children failed to have control of the classroom and did not correct most of the behaviors. On 5/24/2022, children yelled and cried, making it difficult for the surveyor and staff members to hear each other in close proximity. Children ran around the room and wrestled each other. Some children were observed sitting in chairs around a television, which played a children’s program. Some of the children seated in chairs did not appear to be interested in the television program and wrestled each other on top of the chairs. A one-year-old child was observed reaching into a diaper bag and removing the caps to prepared bottles, which were stored in the bag. On 5/25/2022, the lights in the Preschool Room were not turned on upon the surveyor's arrival. There was no sort of engaging programming observed to be offered to the children. Children ran throughout the room and pushed and shoved each other. Several children swung their jackets at each other. Some children kicked over chairs and stood on them. Children screamed and kicked each other. Multiple children climbed into a tall storage cabinet and shut the door. This was not immediately noticed by the staff member supervising the children. A child stood on a chair and swung a large bead maze toy, nearly hitting children who were playing nearby. Children did cartwheels, handstands, threw toys, sat on tables, and jumped over chairs. Two children swung chairs at each other. The staff member supervising the children failed to have control of the classroom and did not correct most of the behaviors. The environment in the Infant Room was chaotic on 5/24/2022. Eight children ages two months to one-year-old were supervised by one staff member. Seven of the eight infants were crying. Two young infants laid crying in cribs. The youngest of the infants (2-months-old) cried loudly and his/her face was flushed and red. The infants were not immediately comforted by the staff member. Three infants, all of whom were crying, sat at a feeding table. None of the infants were buckled into their seats. General: Based on observation, interview and review of documentation, the child care center failed to comply with regulations for background checks. A review of presented staff files and the KARES surveyor report revealed the files for two (2) staff members with the hire dates of 11/18/2019 and 11/11/2019 did not contain a determination of clearance from the Kentucky National Background Check Program, (NBCP) for fingerprinting. It was observed that the aforementioned staff member with the hire date 11/11/2019 was working alongside a staff member in Infant B classroom with eight (8) children in the age range of four-months-old to one-year-old on the date of survey for the timeframe of 10:39am until 1:56pm. Interview with the staff in charge revealed the requested documentation was unavailable at the time of survey. Further observation revealed the aforementioned staff member with the hire date of 11/18/2019 was observed to be working alone in the Three’s classroom caring for seven (7) two and three-year-old children on the date of survey for the timeframe of 10:39am until 11:20am.
