Review of documentation present in the personnel file revealed the following staff member with the hire date of 12/19/2014, did not contain a current copy of a statement from a health professional that the individual is free of active tuberculosis; or a copy of negative tuberculin results. General: Based on Review of Documentation, it was determined the center failed to comply with regulatory requirements regarding general administration, specifically abuse/neglect report procedure.
