Based on review of documentation of a KICCS history note, the surveyor learned that the center would be opening on 4/5/2021. On 04/12/21, the surveyor attempted to conduct a renewal survey; however, the center was closed when the surveyor arrived. The surveyor call the center and left a message to ask for a return call. During interview with staff, the staff reported that she could not remember if she put an opening date in the email to DRCC; however, the center would be opening on 4/19/2021. Based on review of documentation of the email sent to DRCC on 03/23/21, the director informed DRCC that the center would be re-opening on 04/05/21. Therefore, the center failed to provide written notification of a change in hours of operation.
