During interview, staff-in-charge stated that the staff person was seventeen (17) years of age and has not worked alone with children. The surveyor observed the staff person working with children in the Indoor Gross Motor Classroom with another staff (DOH: 10/30/17) that did have a completed background check. Staff-in-charge stated that she mailed the hard copy of the CAN check and has not received a completed copy back as of 12/07/21. The staff's file did not contain a copy of the CAN that was mailed. 2. A staff’s (DOH: 05/22/21) file did not contain documentation of a completed Child Abuse/Neglect Background Check (CAN). During interview, staff-in-charge stated that the staff person was seventeen (17) years of age and has not worked alone with children. The surveyor did not observe the staff person to be at the child-care center on the day of the surveyor’s visit, 12/07/21. Staff stated that she was unsure if she had received the completed copy of the CAN check. 3. Two (2) bottles of DermaCen Hand Sanitizer placed in a gray bucket in an unlocked cabinet underneath the sink in the boys’ restroom. The entry door was locked and no one had a key to enter the building. Upon knocking on the door, the surveyor found that a staff (DOH: 08/1/17) person left a classroom and walked approximately twenty to twenty-five (20-25) feet to open the door. Upon entering the building, the surveyor found that the staff person who opened the door was the only person providing care for seven (7) preschool- age children that were sleeping. The staff person stated that the cook generally assists with the children; however, she had left for the day. When the staff person left the classroom to open the door, the children were left unsupervised for approximately one to two (1-2) minutes.
