Some of children were observed asleep on mats; however, a few children were awake and observed sitting at a classroom table with the staff person. The staff person identified the youngest child present in the group as her six month old grandchild. She stated that a second staff person was working with her in the classroom; however, this staff person had stepped out of the room to use the restroom. The surveyor observed the second staff person enter the classroom approximately one minute later. Since the youngest child in the group was an infant, the minimum staff to child ratio would need to be one staff person for five children and the maximum group size would need to be 10 children. The facility exceeded the staff/child ratio by nine children and the group size by four children. 2. One staff person was observed caring for six children in the infant classroom during the visit. The surveyor arrived at the facility at 9:18AM, and was observing the activity in the children's eating area when a school-age female child opened the exterior door from the playground and attempted to enter the building. At this time, the staff person in charge approached the door and exited onto the playground with the school-age child. The situation was discussed with the staff person in charge during the exit conference at the end of the visit. The staff person in charge stated that the staff person working in the school-age classroom had sent the child over to Building #1 to get paper towels. It is important to note that door the school-age child attempted to enter is located in Building #1 and the school-age classroom is located in Building #2. The school-age child traveled through the Pre-K Classroom in Building #2, exited an exterior door in the Pre-K Classroom that leads to the playground, and then walked through the playground area to access the exterior door she was observed entering. This process/procedure does not provide adequate supervision of the child because there were no staff on the playground at the time and the playground area is not within scope of vision or range of voice from the school-age classroom.
