A purple stationary turtle climber located on the playground was observed to have an approximate one-half (1/2) inch hole in the plastic on the top of the turtle's head. 2. Broken plastic exposing a hole was observed in the bottom of the entrance door into a small yellow/green/blue play house. The broken plastic/hole was observed to have been covered with yellow tape; however, the tape was observed to be peeled off. 3. A small plastic brown play structure was observed to have a crack in the plastic on the middle walkway located over a tunnel.
