General: Based on observation, the surveyor found the following: 1. In the Older Three's Classroom, there was one (1) brown and black purse and one (1) Natural Life bag that contained a prescription bottle of Ondansetron that was located on the window sill; the purse and bag were observed to be accessible to children. During interview, staff-in-charge stated that those items belonged to a staff (DOH:08/12/22) person and that the medication was for her nausea. Staff-in-charge stated that the items had been moved to a locked area. 2. In the Young Three's Classroom, there was a tan and pink purse located on the counter beside the microwave. During interview, staff-in-charge was not sure if the purse belonged to a staff person. 3. In the Young Two's Classroom, there was a four (4) tier shelf that was approximately three feet and five inches (3'5") located behind the classroom door that contained the following: (a) On the second second shelf there was one (1) tube of Desitin Diaper Rash Cream and one (1) tube of Aquaphor Diaper Rash Cream; and (b) Located on the top shelf was one (1) spray bottle of Spic & Span Cleaner, two (2) bottles of Febreze, one (1) tube of disinfecting wipes and one (1) spray bottle of All Purpose Cleaner. The labels for each of the items mentioned stated, "Keep Out of Reach of Children." The surveyor observed each of the items mentioned to be within reach of children. During interview, staff-in-charge stated that those items are usually kept in a locked cabinet and must have been moved. 4. In the Young Two's Classroom, there was a brown cabinet with a broken child safety lock. Inside the cabinet there was one (1) spray bottle of Lysol Disinfecting Spray and the label stated, "Keep Out of Reach of Children." The surveyor observed the item to be within reach of children.
