Based on observation, at approximately 4:10 P.M. a group of seven (7) school aged children left the supervision of the staff in the cafeteria and went into the hallway to gather items, or put items into baskets containing backpacks. The group was unsupervised for approximately four (4) minutes. Based on continued observations, school age children from approximately 4:20 P.M. to 5:50 P.M. randomly roamed out of and into the hallway gathering items or putting items into baskets located in the hallway, ie; based on observation, two (2) school age children left the gym for approximately one (1) minute. Continued observation revealed six (6) other incidences where children walked out of the gym without supervision. Based on continued observation, a class of school age children ran ahead of the staff member into the gym and were unsupervised for approximately two (2) minutes prior to the rest of the group and the staff member arriving.
