General: During a walkthrough of the center, conducted on 12/10/25, the health, safety, and comfort of the children was jeopardized based on the following observations: 1) In the Early Preschool classroom, multiple children wrestled and grabbed each other, climbed on top of and inside of shelves, ran across the room while pushing buckets of toys, and rolled water bottles throughout the floor. The three (3) staff members in the room at the time did not acknowledge or address the behavior, allowing for a chaotic environment within the classroom. 2) A toddler in the Young Toddler classroom tore off pieces of a tissue, put them in their mouth, and chewed them while standing directly beside a staff member who was seated on the floor. Continued observation over multiple minutes revealed that the teacher did not acknowledge the child's actions. The staff member then placed both of his/her hands on the upper left and right arms of the child to roughly put the child on a chair. The staff member scooted the chair up to the table. The child’s chest was touching the table, then the staff member put her leg on the back of the chair so that the child would not be able to get out of the chair. The staff member was yelling at the child on this day. Bruises were found on the child’s upper left arm by another staff member on the day of this incident.
