Based on interview and review of documentation, on multiple occasions a 1-year-old child was allowed access to foods containing dairy despite having a known, severe dairy allergy. In two (2) instances an ambulance was called and one (1) the ambulance calls resulted in him being admitted to the hospital. The center adjusted the menu by adding soy milk but still allowed foods with dairy in the classroom where the child again ingested the food and had a reaction. The child’s enrollment file completed in July 2023 included information regarding the severe allergy, but the center did not develop a plan for handling the allergy until December 2023. The center staff were also not prepared to use the epi pen provided and called the parent instead of 911 during a time the child was having a severe allergic reaction. Based on record review and interview, this regulatory requirement was not met. The facility reported that on 6/17/21, at approximately 10:15AM, a seven-year-old child was upset, ran out of the school-age classroom, down the hallway, and out the front door of the facility. An interview with staff during the investigation revealed that the child ran across the facility parking lot and into a grassy area between the parking lot and a four-lane highway. Staff identified a black street light positioned beside the highway as the area the child made it to before the staff person was able to stop the child from running. The surveyor measured the distance of the area the child traveled during the investigation. The hallway from the school-age classroom to the double set of front doors is approximately 38 feet. The front porch area of the facility is approximately 25 feet. The parking lot area the child would have crossed over is approximately 40 feet and then the child would have traveled approximately 50 feet to the black pole. The child did not sustain any injuries as a result of the incident; however, was in an un-safe area being so close to a busy road. Staff stated further that there are covers for each of the pillows that are washed weekly.
