General: Based on observation and interview, The child-care center failed to ensure electronic viewing material be age appropriate and utilized as an educational tool. The surveyor entered the child-care center at approximately 10:21 a.m. and observed fourteen (14) children ages eight (8) weeks to eight (8) years old grouped together in the indoor gross motor room. The surveyor noted the lights were off and the non-animated Scooby-Doo movie was playing on the television. Some of the older children were sitting and watching the movie while the younger children were walking around the room. The staff person in charge stated they usually watch Super Why but the antenna was broken. The surveyor reviewed daily schedules and found Super Why listed for viewing at 10:30 a.m. However, the surveyor found electronic viewing not reflected on the daily lesson plans. It is important to note the Scooby-Doo movie has a rating of PG for parental guidance, which means some material may not be suitable for children. Additionally, Super Why is a preschool program geared for children ages three-to-six (3-6) years old. General: Based on Interview, it was found that the staff interviewed during this investigation, specifically the assistant director was not truthful with questions being asked in regards to allegations of transportation of children by the center. On 6-5-18 a Division of Regulated Child Care (DRCC) representative interviewed three (3) staff members of the center regarding an allegation the center was providing transportation services. At that time all individuals reported that the center did not transport children. However, on 6-8-18 another DRCC representative completed an annual inspection of the facility and while reviewing documentation provided by staff two (2) invoices were reviewed; one was for $126.82 and the other for $100.00. These invoices were from the facility to two (2) different families for the service of transportation for the month of May. These invoices were dated for 6-4-18 showing they had been made one (1) day prior to the site visit completed by DRCC in regards to an investigation involving allegations of transportation services being provided by the facility without DRCC approval.
