General: Based on Observation, when the surveyor pulled up to the facility, there were nineteen (19) children on the playground with one (1) adult. Another adult came outside as the surveyor was entered the playground area. A third adult followed accompanying a child. The second adult went back inside leaving two (2) adults on the playground. The first adult said she had only been alone for a moment while the second adult took one of the children to the bathroom. The second adult said she walked into the facility and asked for a teacher to go to the playground while she took the child to the bathroom, but she did not wait for a teacher before she went inside.
