General: Based on observation of the playground, the surveyor found the following: 1. A tree that appeared to have fallen and was placed on the ground near two (2) play houses. 2. A white bucket placed beside the back of the providers home that stated "Proform" on the front of the bucket and had a Warning sign on the side of the bucket that stated, "Children can fall into bucket and drown." 3. A loose garden hose placed on the ground near the back of the providers home. 4. A small play house that appeared broken on the playground. During interview, the provider stated that the playground has not been in use by the children due to weather. The provider stated that the tree fell onto the playhouse due to the recent winter storm. The provider stated that she would have the items mentioned moved as soon as possible. Covers had been placed over the windows to shield the light in the rooms that are used by the children at nap time. The rooms were too dark to see the children, creating a risk for their safety. 2. The surveyor found that a plunger had been placed beside the commode in the restroom that is used by the children. The plunger was accessible. 3. The surveyor observed that laundry had been piled on the floor in front of the washing machine. Through interview, the surveyor learned that the laundry was soiled and was accessible to the children. 4. The surveyor found that a tile was hanging down from the ceiling located near the crib where an infant child sleeps. 5. The surveyor observed that portions of the sheet rock had been removed in the restroom that is used by the children.
