General: Based on observation, while conducting the initial headcount at the child care center, the surveyor observed seven (7) children being supervised by one (1) staff member in the Younger One's Classroom. During interview, staff-in-charge confirmed that all the children in the classroom were one (1) year old; therefore, the classroom exceeded the staff-to-child ratio by one (1) child. Staff- in-charge confirmed that the classroom was out of ratio due to a staff member leaving due to illness. Staff-in-charge informed the surveyor that a substitute been contacted to replace the staff member.

General: Based on observation, the surveyor discovered the following: 1. A plunger was placed on the floor beside the toilet in the older pre-K restroom. The plunger was accessible to the children. 2. In the older pre-k restroom, children's tote bags containing blankets were hanging on hooks and were touching; thus, creating a potential for cross-contamination. Therefore, the health, safety, and comfort of the children were compromised.

General: Based on review of documentation, the surveyor found the following: 1. A staff’s (DOH: 06/25/25) file contained a negative tuberculin verification that was no longer current after 08/21/23. 2. During interview, staff-in-charge confirmed that she was aware that the TB skin tests were no longer current and informed the surveyor that the staff members were scheduled to have them renewed this month.
