General: Based on observation, the surveyor found the following: 1. A container of Bleach that read, "Keep Out of the Reach of Children," on the label was placed in an unlocked cabinet under the sink in the restroom. 2. Two (2) lawn mowers located next to the sidewalk and by a set of steps that lead up to the entry door of the Child Care Room. 3. One (1) weedeater placed on the ground by the provider's home and near the sidewalk that leads to the Child Care Room entry door. 4. A piece of wood that contained the flat end of two (2) exposed nails was observed sticking up out of a garbage can that was not covered properly. The garbage can was located near the sidewalk that leads to the entry door of the Child Care Room. The items mentioned were observed to be accessible to the children. A piece of metal skirting on the bottom of the provider's home was observed to be detached exposing an approximate six (6) inch gap and one (1) rusty nail located beside a set of steps. 2. A piece of metal skirting on the bottom of the provider's home was observed to be detached exposing an approximate three and one-half (3 1/2) inch gap located on the end of the home by a small play structure. 3. A red metal pole was observed sticking up through part of the back corner fence roll on the playground. General: Based on observation of the playground, the surveyor found the following: 1. The front part of a water table that was chipped/cracked located on the mulched area of the playground. 2. A long metal pole located on the mulched area of the playground. 3. A red metal bar that was attached to a slide on the mulched area of the playground contained two (2) rusty holes that appeared to have sharp edges. 4. A long silver metal pole that was zip tied at the top of the pole to the fence located beside a building on the playground. The pole was observed to hang down on the fence and contain a rusty end. 5. The top metal pole to the chain link fence located on the side of the playground by a small plastic play structure was observed to be zip tied to the top of the fence and the end of the metal pole was observed to be sticking out through the fence. 6. A metal pole was observed to be on the other side of the fence located by a plastic play structure and was propped up against the fence. The pole appeared to be holding the fence up and the end of the pole was observed to come through the fence. The end of the metal pole had an opening that had one (1) exposed nail. 7. The bottom wooden step that leads from the provider's home onto the playground was broken and exposed one (1) nail.
