General: Based on interview, the following was found: On 12/27/24, during naptime at approximately 1:20 pm, a staff member was in the restroom assisting a child. She and the child were in the restroom for approximately twenty (20) minutes. The other eleven (11) children were asleep on their cots. The staff member did not have view of the classroom while she and the child were in the restroom. After she and the child exited the restroom; she realized one (1) of the children, a three-year-old male child, was not on his cot nor in the classroom. She vocalized that this child was not in the classroom. The staff member in charge and another staff member searched for the child in the building. The staff member in charge walked to the front of the building and saw two (2) police officers and an adult female in the parking lot. The adult female was holding the male child. The staff member in charge told the police officers and the adult female that the child belonged with her. The staff member stated that the adult female saw the child roaming around the Park Lane Shopping Center for approximately three (3) minutes before she knocked on the child-care center’s door. After no answer, she carried the child across the parking lot to Highland Fitness Center to an off-duty police officer. Based on review of documentation, the off-duty officer called for assistance at approximately 1:57 pm. The child was returned to the staff member in charge at approximately 2:02 pm. Observations made while touching the exterior of the sippy cups revealed that the contents of the cups were room temperature; revealing that the cups had not been stored at forty (40) degrees Fahrenheit or below. Continued observations revealed a staff person proceeded to pour additional milk into several of the cups and then serve the cups of milk to the toddlers for snack. Ongoing observation revealed the director provided instruction to the staff person to rinse the cups prior to re-serving them to the children in care. Observation revealed the staff person dumped the left over milk from the labeled cups, rinse the remainder of the sippy cups in the classroom sink, used to wash hands after diaper changes, and refill the cups with milk prior to serving the milk to the children in care. The infant was observed crying and sleeping intermittently during this time, without attention from a staff member to offer comfort or care. Ongoing observation in the Infant / Toddler Room revealed an infant sleeping directly on a crib mattress without bedding and toddlers sleeping directly on plastic napping mats without bedding. Further observation in the Infant / Toddler Room revealed a toddler crying during the entire survey, failing to receive comfort from the staff present. Continued observation revealed the toddlers in the Infant / Toddler Room were served milk from a container that expired on December 18, 2014 and had a distinct sour odor. The service was stopped by the surveyor prior to any of the children consuming the spoiled milk. Ongoing observation in the Infant / Toddler Room revealed a child’s coat and diaper bag left in the floor at approximately 10:00 a.m.. Continued observation revealed a child fell over backwards on the items at approximately 11:28 a.m.. Further observation of the Infant / Toddler Room revealed a staff person transferring an infant from the diaper changing table to the floor by one arm and a staff person transferring a toddler from a high chair to the floor by one arm; failing to maintain the comfort and safety of the children.

The child began to verbally express that he wanted to leave the room as the staff member tried to verbally redirect him in a calm tone to remain in the room to assist her. At approximately 11:49 am the five year old child walked to the door and proceeded to open the door to leave the room. Continued observation revealed the staff member left the area that she was standing at which allowed her to visually see both areas to prevent the child from leaving the room. Further observation revealed the six children in the Preschool Area were left unsupervised until approximately 11:51 am. At approximately 11:52 am the five year old child in the School Age area bolted out of the room with the staff member immediately following behind again leaving the six children in the Preschool Area unsupervised.
