General: Based on interview, a three-year-old child fractured her leg after falling while on a bounce house at an off site parent involvement field trip at a bowling alley. Staff interviewed stated that the child's parents were not on site at the time of the incident. Staff stated that after several attempts to reach the child's parents, staff took the child in a personal vehicle to the hospital. The center is not licensed by the Cabinet to transport children.

General: Based on observation, while conducting the initial head count at the child care center, the surveyor found the following: 1. There were seven (7) children present in the Toddler Room that measures for a maximum capacity of five (5) children. The classroom capacity was exceeded by two (2) children; therefore, there was not thirty-five (35) square feet of space available per child. 2. There were four (4) children present in the Nursery that measures for a maximum capacity of three (3) children. The classroom capacity was exceeded by one (1) child; therefore, there was not thirty-five (35) square feet of space available per child. During interview with staff-in-charge, she stated that she was unsure of the approved capacity for each classroom. The surveyor reviewed the diagram with the staff-in-charge and informed her of the approved DRCC capacity for each classroom, noting that two (2) of the classrooms exceeded the maximum capacity allowed. Staff-in-charge stated that some of the children counted in the over-capacity rooms did not belong to those classrooms. The surveyor explained that regardless of the assigned classroom placement, the children were physically present and observed in the classrooms for the duration of the surveyor's visit and were counted toward each classroom's capacity.
