General: Based on observation, the surveyor found the following: 1. In the Child Care Room, there was loose telephone cord coming out from the wall and laying across the floor. While the surveyor was reviewing paperwork, the provider did remove the loose telephone cord. 2. In the Child Care Room, there was a speaker attached to the wall that had loose cords hanging down that were laying across the floor and underneath the classroom table. 3. In the Child Care Room, there was a T.V. mounted to the wall that had a loose cord hanging from the television across the floor. While the surveyor was reviewing paperwork, the provider did remove the loose cord. 4. While the surveyor was reviewing paperwork, the surveyor observed seven (7) children from 1:50 p.m. to 2:00 p.m. sitting in plastic chairs and taking taking turns running / pushing each other into other children while screaming. 5. When the surveyor arrived at the child-care center, the provider informed the surveyor that seven (7) children were present. The children who were in the Child Care Room began to shout there was actually eight (8) children present and that one (1) child was hiding upstairs. The provider stated that the child upstairs was her daughter's son and was not enrolled in the child-care center. While the surveyor was reviewing documentation, a parent showed up to pick up two (2) children. The surveyor observed one (1) child leave the Child Care Room and the surveyor observed from a downstairs window a child leave from the upstairs side door. The surveyor again asked the provider whose child that was and she stated that he was not her daughter's son, but her daughter was watching the child for the day. The surveyor asked the provider if she had any documentation on the child that was upstairs and the provider could not provide any documentation. 6. When the surveyor returned back inside after checking the outside play area, the surveyor heard a faint cry. The surveyor walked back downstairs toward the Child Care Room and found a bedroom door that had the door slightly opened. The surveyor opened the door to find a two (2) month old infant in a car seat sitting on a bed. The surveyor asked the provider who the baby belonged to and she stated that the baby was her daughter's child. The surveyor asked the provider if she had any documentation on the infant and the provider could not provide any documentation. General: Based on observation, the surveyor began overhearing children stating at 2:00 p.m. that they were hungry and starving. The surveyor also overheard a child state that they had not had lunch. A child asked the provider when would she would be serving food to the children. From 2:00 p.m. to 3:10 p.m., children were observed talking amongst themselves about how hungry they were, talking about what food they would sneak into the providers home tomorrow to eat and asking why they had not ate lunch yet. When the surveyor asked the provider if the children had ate lunch, she stated that the children had ate and a child stood up and yelled, "no she did not give us lunch, when are you going to feed us?". The provider finally stated that the children had not ate lunch and advised the children would be eating snack at 3:15 p.m. The provider stated that the children had ate breakfast at 8:30 a.m. that morning; therefore, the child-care center failed to provide food every two (2) to three (3) hours to the children. General: Upon arriving at the certified family child-care home, the surveyor knocked on the door and overheard a female’s voice inside say “Stay downstairs, stay downstairs.” The surveyor was met at the door by the assistant (DOH 7/30/18), who advised the surveyor that the provider was not present at the residence. The assistant stated no children were present and no children would be there until after 3:00 p.m. The surveyor left the premises at approximately 11:09 a.m. and returned back to the residence at 11:20 a.m. Upon arrival to the certified family child-care home, the surveyor observed the assistant get out of her car and briskly walk back into the house. When the surveyor knocked on the door, the provider opened the door and immediately closed the door back while the surveyor was attempting to gain entry into the certified family child-care home. After this occurred the provider opened the door and allowed the surveyor to enter the residence. Upon entry, the surveyor observed a total of four (4) children in the care of the provider. During interview, the provider alleged that she had just returned home and confirmed that the children were present during the surveyor’s first attempt to enter the home. The surveyor asked the provider who was caring for the children while she was out of the home. The provider advised the surveyor that the assistant was caring for the children during her absence; therefore, the assistant did not allow the surveyor access to the certified family child-care home while children were in care.
