Based on interview the surveyor found that an individual who the child care center identified as a "volunteer", was acting in a supervisory role and had been left alone with children. Upon review of documentation in the "volunteer's" file, the surveyor found a completed staff records cover sheet indicated by checkmark, that a KARES final registry form was on file. However, the surveyor did not observe the form to be on file. The surveyor further observed that the file contained additional completed employment-related documents, including a completed W-4 form, a completed direct deposit authorization form, and a signed employee drug policy acknowledgment.
