General: Based on review of documentation, the surveyor found that a staff member (DOH: unknown) whom the director identified as a current employee was listed as separated with an eligibility date of 04/02/25 and closed determination status on the center’s KARES roster printed the morning of the inspection. The surveyor observed this staff member working alone with twelve (12) children aged five (5) years to twelve (12) years old in the school age classroom during walkthrough of the center. The director stated that this staff member had worked for the center previously, left employment in February 2026 and returned in late April 2026. She stated that she was unsure of the exact date of re-hire as the computer at the center was unable to bring up her KARES account, and there was no date of hire recorded in this staff member’s personnel file. The surveyor's subsequent review of this staff’s applicant history in KARES revealed a hire date of 05/13/25, separation date of 08/22/25, and determination closed date of 04/08/26. Therefore, the surveyor was unable to verify that the staff member had submitted updated background results to the center prior to re-employment with the center and more than one-hundred and eighty (180) days since the staff member was separated on KARES.
