General: Based on review of the Kentucky National Background Check Service, the surveyor found that a staff’s (DOH: 11/01/21) employment status was listed as “Provisional” and the determination status was listed as “Closed-Fingerprints not Taken.” The surveyor found that the staff person submitted to fingerprints on 05/04/18 and the fingerprints were rejected by the FBI on 05/09/18. The surveyor found that the staff person submitted to fingerprints again on 05/17/18 and the fingerprints were rejected by the FBI on 05/24/18. The surveyor found that the staff person was provisionally hired on 11/01/21 and a Final Registry Results letter was generated on 11/01/21.
