The surveyor was in the center's parking lot at 9:00 am on 05/08/25 and observed a two-year-old child with his head, part of his upper body and both arms hanged outside of an open, unscreened window. Upon entering room 2A, the child was observed standing on a chair with his head and arms still outside the open, unscreened window. The responsible staff member was observed at the diapering station diapering another child. Interview with the responsible staff member revealed the window did not have a lock; therefore, the child opened the window several times daily.
