General: Based on observation, interview and review of documentation, a seven-month-old infant received a 2x2.5cm second-degree burn to her chin on 05/03/19. Interview with the responsible caregiver determined the plastic five (5) ounce Nuk Bottle containing four (4) ounces of formula was placed into the water filled bottle warmer for approximately eight (8) minutes. The bottle was removed from the bottle warmer, but the caregiver did not test the temperature of the formula before she fed the bottle to the infant. The infant immediately began to cry when the nipple was placed in the infant’s mouth. The caregiver removed the bottle’s nipple from the infant’s mouth and some of the formula dripped onto the skin of the infant’s chin. The caregiver then used a washcloth to wipe the formula off the infant’s chin and noticed a layer of sloughed skin on the chin which left the infant's chin raw, wet and red. The caregiver documented the injury/accident report as a carpet burn. The infant’s mother took the infant for medical treatment and a physician diagnosed the infant with a second-degree burn and not a carpet burn. The caregiver later retracted her documented injury/accident report and admitted on 05/06/19, during an interview with a local police detective and a representative with the Department for Community Based Services (DCBS), the injury was received because very hot formula dripped from the bottle onto the infant's chin. Surveyor tested the water in the bottle warmer on 05/07/19 and determined the water began to boil at two (2) minutes. At four (4) minutes, the water registered at two hundred and ten (210) degrees Fahrenheit and at eight (8) minutes the water registered two hundred and eighteen (218) degrees Fahrenheit.
