General: Based on review of documentation, the surveyor discovered the following: 1. A staff member (DOH: 03/24/25) did not have a completed KARES report; therefore, the surveyor was unable to determine if the staff member was eligible for hire. The staff member was not working during the survey visit; however, during interview with staff-in-charge, she stated that the staff member had worked with children at the child-care center. 2. A staff member (DOH: 04/21/25) did not have a completed KARES report; therefore, the surveyor was unable to determine if the staff member was eligible for hire. During interview with the staff-in-charge, she stated that she was prompted to complete a CAN check on the staff member since the staff member had not lived in Kentucky long enough. The staff member was observed working alone in the Toddler Classroom on the day of the surveyor’s visit. The staff member was supervising four (4) children that were eighteen to thirty-six (18-36) months old. 3. A staff member (DOH: 06/18/25) did not have a completed KARES report before working in the child-care center. The staff member was listed as "in progress” on KARES; therefore, the surveyor was unable to determine if the staff member was eligible for hire. The staff member was working alone in the Preschool Classroom on the day of the surveyor’s visit. The staff member was supervising four (4) children that were three to four (3-4) years old.
