General: Based on observation, the surveyor found the following: 1. In the Preschool One Classroom, there was one (1) electric outlet cover missing beside the long table. 2. In the Preschool One Classroom, there was a loose cord hanging from a pencil sharpener that was on top of a tall shelf; the cord was plugged into the wall and was not secure. 3. In the Older Two's Classroom, a bottle of Eucerin Daily Hydration Lotion and a bottle of Bello Hand Sanitizer Gel had been placed on top of a small shelf. The items contained a label that read, "Keep Out of Reach of Children" and both items were observed to be within reach of children. 4. In the Older Two's Classroom, the surveyor found two (2) rodent glue trap's behind two (2) toilets in the upstairs rest room. The surveyor observed both rodent glue traps to be accessible to children. 5. On the playground, there were two (2) five (5) gallon buckets that read "Big Blue Lubricant" on the buckets. The labels on buckets read, "Keep Out of Reach of Children." Both buckets had been placed on the patio on the playground and both buckets had the lip removed. While observing the buckets, the child-care center cook came outside and informed the surveyor that the buckets actually contained salt for melting ice from the ice storm a few weeks ago and the child-care center had not put the contents up yet. The surveyor observed the buckets to be filled with salt and observed there to be salt on the ground around the buckets. The salt and the buckets were observed to be within reach of children and observed to be accessible to children; therefore, the buckets of salt caused a threat to the health, safety and comfort to the children at the child-care center.
