Based on observation, a large climbing structure had a piece of blue protective covering approximately 3-4 inches long, missing from an area at the top of the chain climbing ladder, revealing rusted iron, and having rough, sharp edges. A second large climbing structure also had blue protective covering approximately 6 inches long, missing at the top of the climbing steps. The area exposed rusted and corroded iron. Based on continued observation, on the tubular slides on both climbing structures, were approximately 28 circular discs approximately two inches in circumference of broken plastic with sharp, rough edges. Approximately 9 of the circular discs were missing completely, leaving the equipment in disrepair.

General: Based on Review of Documentation, the child care center failed to maintian children's records in accordance with regulatory requirements. Based on a review of ten children's files, a child enrolled on 03/07/14, failed to have contact information to enable a person in charge to contact the child's physician. Based on continued review, children enrolled on 10/06/14, and 10/06/14 failed to have contact information enabling a person in charge to contact the child's physician, the child's general health status and medical history, and also failed to have an authoriziation by the parent for the child care center to seek emergency medical care for the child in the parent's absence.

General: Based on Interview and review of documentation the child care center did not maintain compliance with regulatory requirements. Review of the center's posted license which had expired on 08/31/2013 revealed transportation was an approved service. Continued review revealed the center's current posted license which was to expire on 08/31/2014 did not have transportation listed as an approved service. Interview with the staff in charge and further review of documentation revealed a field trip had been taken on 10/10/2013 where the center had arranged transportation through a third party. Ongoing review of a copy of the renewal application originally submitted by the center on 06/03/2013 but was returned to the center as being incomplete revealed the application was returned by the center on 07/29/2013 with transportation services having not been requested.
