General: Based on Observation and interview, the child care center director failed to assure compliance with regulatory requirements. Upon the surveyor's entry at approximately 10:15 A.M., a preschool age female was observed lying on the floor. Interview with the staff member in the classroom revealed that the child was sick and was experiencing a stomach ache, but may be picked up by a parent soon. At 10:30 A.M. the preschooler was still lying directly on the floor, failing to have a mat or a cot under her, using a cloth rug (mat, as stated by the staff member) for a cover. Interview with the staff member revealed that the child was lying on the floor as "nap time is at 12:00 and it is already 10:30". When asked why the child was lying directly on the floor rather than on a cot, the staff member stated that the child's cot "is too far down in the stack", and continued stating that the child had been lying on the rug but chose to cover with it rather than lie on it. General: Based on Observation, the child care center director had not assured compliance with regulatory requirements. Observation during lunch time revealed a pair of safety scissors had been placed in a cup at each table for the children to use in opening up packaged food items. Continued observation revealed a male child was taking the scissors and tapping his forehead with the handle portion of the scissors before passing them to a second male child who was opening and closing the scissors. Further observation revealed the scissors were then passed to a female child who attempted to cut the edge of the table before handing them to a fourth child who used the scissors to cut open a tube of yogurt. Ongoing observation revealed the four staff members in the room at the time were assisting other children and did not redirect the three children that were playing with the scissors inappropriately.
