General: Based on Interview, on 8/8/13 a fifteen (15) month old child received an injury to the distal end of her fifth digit on her left hand while in care at the facility. The child's finger had been slammed in a door in the Toddler classroom. In interviews with staff present at the time of the incident it was found that the finger was bleeding and that the end of the finger was "barely hanging on". According to the classroom staff person the child was crying and there was blood on the child’s clothing, the staff person’s hands and down the front of the staff person’s leg. The staff person in charge gave the classroom staff person a paper towel to wrap around the child's finger. Both staff present were currently certified in First Aid, however, the paper towel was only treatment given to the child. The staff person in charge stated that she went to her office and called the child’s parent. She indicated that she did not call 911 because the child's mother worked close by and she thought the child's mother would get to the facility before emergency services. When the child’s mother arrived she told the staff person in charge to call 911 and let them know she was on her way with the child and what the injury entailed. The child’s mother asked the staff person in charge to accompany her to the hospital so she could ensure that the child did not pull off the end of her finger on the way. Due to construction and heavy traffic, the child’s mother pulled into a parking lot and called 911 because the child was still bleeding. Fire Department paramedics treated the child’s finger in the parking lot and an ambulance took the child to Kings Daughters Hospital in Ashland, KY. The child was transferred to Cabell-Huntington Hospital in Huntington, West Virginia for surgery. The child’s finger was completely severed between the first joint and the nail bed and could not be re-attached. An eleven (11) month old child was observed sleeping in a bouncy seat in a crib. The staff present stated that the child had a physician's statement indicating that the child was to be propped while sleeping due to acid reflux disease.
