After a tour of the lower and upper levels of the building, the staff person in charge started asking questions about paperwork that the surveyor would need to review. At this time, the surveyor asked if the facility operates in a second building because the surveyor remembered the building from a previous inspection for another provider several years ago. The staff person in charge replied yes, and indicated that the Pre-K Classroom and the School-age Classroom are housed in building #2. The surveyor asked the staff person in charge for a tour of the space. The staff person in charge stated that they typically drive to the second building; therefore, the staff person in charge got in the vehicle with the surveyor and drove .1 mile up the road to 300 Washington Street. The surveyor observed 13 children (four- to five-years-old) and one staff person present at this location.
