General: Based on interview and review of documentation, on 10/20/25 a two-year-old child turned a chair upside down was trying to stand on top of it and fell hitting her forehead on the floor. Staff stated that she checked the child over and at the time saw no marks on her head and the child went back to playing. Staff stated that she never told the office or the parent about the incident. Pictures sent through the center app to the parent around 2:30pm show bruising and marks on the child's face but staff still did not reach out to the office or parent. Child was taken to the hospital the next morning due to not eating and throwing up. The child was diagnosed with a concussion. Employee Records In Compliance
