General: Based on review of documentation, the surveyor found that a staff’s (DOH: 10/02/25) file did not contain documentation of a background check that had been submitted through KARES and the surveyor found that the staff person mentioned was not listed on the child-care centers current KARES Report staff roster. Upon review of the child-care center's KARES Report, the surveyor found the staff person listed as "separated." The surveyor observed the staff person to be working with children on the day of the surveyor's visit. The surveyor was able to verify once the surveyor returned to the regional office that the staff person mentioned had been separated from KARES since 08/22/23; therefore, the staff person was working with children while being separated from KARES. The surveyor did observe that the staff-in-charge rehired the staff person on 12/29/25 at 1:07 pm. This was after the surveyor had left the child-care center. The surveyor also confirmed via KARES that the staff person was eligible for hire as of 03/28/23.
