The provider admitted she had a general transportation form that was signed that was contained within the packets. The signed forms were observed in the files. The forms state that the parents will be notified beforehand of each outing, the provider did not do this on an individual basis for each trip and the parents were unaware of the field trips that were taken. The provider now has a portal system that she notifies the parents where they are going for any of the field trips and the parents are aware.
