The provider had a door that as not o the hinges that when the consultant went in the room fell onto the consultant. There was a mop bucket in the kitchen that had water to the top that was not attended to and openly accessed to the child in the home that was a crawling infant that could fall into the bucket of water. There is tiling inthe kitchen that is pealed back that the consultant tripped over in the kitchen area. The backyard has not been mowed and is very high at this time which can cause issues for the children to play outside.
