Program self-reported two incidents in P2 that occurred on 4/14/26. Teacher used physical punishment two different times to correct unacceptable behaviors. One incident the teacher contained child on her lap, the other incident she held child between her legs while she was in a chair and child was on floor.

Program self-reported an incident that occurred on 5/13/25 involving a caregiver not using positive discipline with a five year old child. Caregiver did not communicate the desired behavior to child in a calm manner.

Program self-reported an incident involving alleged abuse by child care center personnel to DCS on 5/14 at 5:10 pm, but did not report to the division until 48 hours later on 5/16 at 5:36 pm. Parents were not notified of full situation involving their child.

Program self-reported an incident on 5/13/25 involving a caregiver in preschool 3 classroom. Caregiver used inappropriate discipline with a 5 year-old student. Teacher yanked child off floor in a harsh and frightening manner, then restrained child unnecessarily.
