    After the parent notified the program of the fracture, the program generated an incident report; however, the report was back-dated to reflect the date of the injury rather than the date it was completed. During interviews, staff who were present at the time of injury confirmed that they did not verbally notify the parent or guardian and acknowledged that the child complained of pain throughout the day and was observed limping. Staff reported being present at pick-up, with one staff member, stating she “just forgot” to inform the parent of the injury.
