Some items labeled "keep out of reach of children" were accessible to children: The laundry room was unlocked and contained stain remover and purex laundry detergent. Contact solution and shaving cream were located on a low shelf in an unlocked yet deterred cabinet in the two's room. Lysol wipes were being stored under the unlocked yet deterred diaper table in the infant 2 room. In the teacher desk of the preschool room, there were adult scissors, carmex, lotion, and prescription pills.

On 9/11/2024 the program self-reported to licensing and DCS that a 2 year old female child bit a male 2 year old child. To separate the children, the teacher grabbed the female child by arm in a forceful way.

There were several "keep out of reach of children" items accessible to children throughout the building. In the second toddler room, there was a first aid kit on a low shelf of an unlocked cabinet. In the school age room, there was spray paint and hand sanitizer on a low shelf of an unlocked cabinet. In the unlocked kitchen, cleaning chemicals were out in the open stored on the floor.

On 9/8/2023 at approximately 4:00pm a teacher grabbed a male school age child roughly by the arm leaving a red mark as well as tearing the skin. This was captured on video and teacher was immediately terminated.
