470 IAC 3-1.2-5(a) · Licensee shall provide a separate crib, portacrib, or playpen in which each infant or toddler can sleep. Each crib, portacrib, or playpen shall be equipped with a firm-fitting mattress or pad made of waterproof materials. A parent or legal guardian may provide or approve the use of a bassinet for an infant and is responsible along with the caregiver to monitor its use closely. (Manufacturers of bassinets indicate that a bassinet should no longer be used once an infant begins moving and turning unassisted.)
During the PTQ rating observation both infants present were laid down for nap in their own pack-n-plays. One child was nine months old and the other was eight months old. Each had a pacifier with a beaded clip still attached while they slept.

Otherwise the ratio is 4:1. Mixed Age Groups (Birth-6 years) 10:1* *No more than three (3) of the ten (10) children may be under sixteen (16) months of age and must be walking. 3 years and older (3-10 years) 12:1
Upon arrival at the home, 12 children were present with one qualified caregiver. With the ages present, two qualified caregivers must be present at all times. To have a ratio of 12:1, all children must be 3 years or older. Another staff member did come during the visit. The consultant left with the home being in ratio.
