The child was discovered only after the child’s grandparent contacted the center to request a change in the child’s pick-up and drop-off arrangements. At the time the child was located, the child was not under direct supervision as required by licensing standards. The center reported did not report this incident to the Department of Child Services (DCS), despite mandatory reporting requirements for any incident involving a lapse in supervision or a child being left unattended in a vehicle. Failure to report the incident constitutes noncompliance with required reporting and supervision regulations intended to ensure child safety.

On April 28th, 2026, the program failed to maintain supervision of children in care. During interviews conducted with the director and staff present that day, it was reported that a child was left on childcare bus after pickup from school to daycare. Staff reported they did not observe anyone else on the bus and were unaware of the incident until the grandparent called to discuss pickup and drop off authorization.
