Door going to basement was not latched. There is not a latch on door going to upstairs, both doors need to be latched during child care hours. Four tubes of toothpaste and first aid kit with lotions that say Keep Out of Reach of Children under bathroom sink.

At time of consultant's arrival a seven month old male was asleep in a baby bouncer, with two blankets and a pacifier on a pacifier on a cord. There was also a loose long charger chord from bouncer in seat of bouncer under infant.
