470 IAC 3-4.5-6(d) · Diapers. The diapering process shall be done on a table, in a clean and sanitary manner. The diaper changing surface shall be sanitized after each use and materials used for skin cleansing shall be discarded after each use into a tightly covered, easily sanitized container. Individuals responsible for diaper changing shall wash their hands after each diaper change
In room D-12 the diaper table had diapers spread across it. In room D-8 their was a box of Diapers on top of changing pad. Diaper tables should not be used for other purposes.

In room D-12 a can of air freshner reading keep out of reach of children was within the child's reach. In room D1 & 2 a can of sunscreen was in a back pack accessible to children in the cubby. In room D-5 in restroom on shelf was one diaper and on top of storage unit were open diapers. In the upstairs Girls RR a roll of toilet paper was sitting on top of the tampex machine. All items in restroom are to be sanitizable.

470 IAC 3-4.5-4(2) · All restrooms shall be equipped with flush toilets and handwashing sinks and shall be ventilated to the outside. An adequate supply of water, under pressure, shall be provided at all handwashing sinks, as well as soap and disposable paper towels in dispensers. Room D-11 had 6 different items (Baby Powder, eczema, etc.) in drawer accessible to children that states keep out of reach of children. In room D-1 multiple pairs of scizzors were on top of teachers desk accessible to children which is not safe. In room C-7 6 bottles of sunscreen were on shelf accessible to children and states keep out of reach of children. Room C-10 had 4; D-11 had 2; D-8 had 1 missing outlet cover that is not safe.
