On June 8, 2026, at 2:08 PM, staff assigned to the infant room reported that an infant was accidentally given another infant’s bottle of breastmilk. The staff member stated that the infant consumed approximately two ounces before the error was identified. Ratio was not maintained. During the walkthrough on the date of inspection, the Pooh Bears classroom was observed to have ten children, with reported ages of 12 months and older, supervised by only one caregiver. This staffing pattern does not meet required child-to-staff ratios for this age group. Additionally, nap-time ratio requirements were not met. The Compliance Specialist could only confirm that 2–3 children in the classroom were asleep, which did not meet the minimum requirement of 50% of the children being asleep in order for the room to operate under reduced nap-time ratios. Because fewer than half of the children were sleeping, the classroom was required to maintain the full awake-time ratio, which was not achieved.
